The Chamber of Commerce and Industry of Ukraine explains which package of documents must be submitted in order to obtain a certificate of force majeure:
Appropriate application form. The application for the certificate of force majeure circumstances shall be submitted under the signature of the head of the enterprise, the organization with the imprint of the enterprise seal, or on his behalf – by a proxy.
Copy of the contract.
A copy of the Order in connection with the necessity to cease activities on the basis of the relevant legal acts.
Message (notification of other party).

Please also note that the applicant has the right to apply to the Chamber of Commerce and Industry of Ukraine for a certificate of force majeure in the event of a breach of the obligation due to the quarantine entered.
